Palliative care is specialized medical care for people with serious illness. Appropriate at any age and at any stage in your illness. The goal is to relieve symptoms and stress and improve quality of life.

- A collaborative approach, working together with your primary care provider (PCP) to make complex decisions about medical treatments that reflect your preferences

- A dedicated team including a hospice physician, registered nurse, social worker and chaplain — with support from additional nursing staff, pharmacists, physical therapists, massage therapists, volunteers and others as desired by the patient (such as music therapists)
- Pain management
- Developing your advance care planning documents
- Coordinating your care among multiple care settings
- Addressing you as a whole person regarding concerns about your illness
